Join Southeastern Integrated Care LLC as a CST-Para Professional and be part of a dynamic team that provides support programs and care to enhance the quality of life for individuals with behavioral or mental health challenges and/or substance abuse issues.
Job Summary:The CST-Para Professional is responsible for providing direct treatment and restorative interventions, as well as case management, to individuals in their homes and communities. This role requires a strong ability to interact effectively with consumers, promote relationships with family and friends, and support community-based opportunities.
Key Responsibilities:- Provide direct support and care to individuals in their homes and communities, including assistance with daily living tasks and medication management.
- Develop and implement individualized treatment plans, monitor progress, and make adjustments as needed.
- Collaborate with community partners, including social workers, instructors/coaches, and day program staff, to ensure comprehensive support services.
- Assist with crisis interventions, provide psychoeducation, and support individuals in attending professional appointments and life skills training.
- Ensure linkage to community services and resources, and assist with the development of relapse prevention and disease management strategies.
- Participate in team meetings and special events, and contribute to the development of the Person-Centered Plan.
- Document all billable encounters and maintain accurate records, including incident reports and medication administration records.
- Minimum one year experience supporting adult individuals with behavioral or mental health challenges and/or substance abuse issues.
- High school diploma, GED, or equivalent.
- Ability to write clearly and legibly, and provide guidance and training on life skills.
- Knowledge of behavior management techniques, substance abuse counseling/support, and recreation leadership.
- Ability to interact effectively, including oral and written communications, and provide a pleasant, positive, and nurturing personality.
The CST-Para Professional will work primarily in the client's home and in the community, with occasional travel to team meetings and training sessions. The work environment may involve exposure to cigarette smoke, domestic animals/pets, household pests, and other issues related to the domestic location in which service is delivered.
Physical Demands:The CST-Para Professional will be required to stand, walk, use hands to finger, handle, or feel objects, tools, or controls, talk and hear, and taste and smell. The employee will frequently be required to reach with hands and arms and stoop, kneel, crouch, or crawl. The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
