Key Responsibilities

We are looking for a highly skilled and compassionate Registered Nurse to join our team as a Home Therapies RN CAP 1. In this role, you will:

  1. Assess patients' physical and emotional needs, develop individualized treatment plans, and coordinate care with other healthcare professionals.
  2. Provide ongoing education and training to patients and their families on dialysis therapy, medication management, and lifestyle modifications.
  3. Work collaboratively with the interdisciplinary team to achieve optimal patient outcomes and quality of life.
  4. Participate in on-call rotation, night, weekend, and holiday shifts as required.
  5. Perform all duties under the direction of the Supervisor, with guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse.
