Healthcare Manager with Chronic Disease Expertise

Bluestem Health is seeking a skilled Registered Nurse to serve as a Case Manager. This role will focus on educating patients with chronic diseases, including behavioral health diagnosis, and providing case management to individuals with complex medical/behavioral health illnesses.
About the RoleThis position requires a strong understanding of healthcare management principles and the ability to develop effective plans of care. The ideal candidate will possess excellent customer service skills, be highly organized, and have a self-starter mentality.
Key Responsibilities- Promote teamwork and enhance a positive work environment by encouraging collaboration and mutual respect among coworkers.
- Develop and lead the development of chronic disease program protocols and standards of care.
- Provide ongoing supportive and/or case management functions in accordance with patient needs and strategies identified within the service plan.
- Maintain close communication with consulting providers, nursing staff, and behavioral health consultants for input regarding medication compliance and patient adjustments.
- Serve as liaison between patients and staff, ensuring seamless communication and coordination.
To be successful in this role, you will need:
- A Registered Nurse license in the State of Nebraska (BSN preferred)
- Certified - Basic Life Support
- At least 3 years of Nursing experience (preferred)
- Excellent problem-solving skills, computer skills, and the ability to meet deadlines
The estimated salary for this position is $85,000 per year, based on industry standards and market research. Additionally, Bluestem Health offers a comprehensive benefits package, including medical, dental, and vision coverage, as well as a 401(k) retirement plan and paid time off.
