  • Collaborate with the interdisciplinary team to identify and address patient needs, ensuring seamless care coordination and communication.
  • Develop and maintain relationships with community resources and organizations to provide patients and families with access to necessary support services.
  • Participate in performance improvement activities and contribute to the development of policies and procedures to enhance patient care and outcomes.
Requirements
  • Licensed by the Board of Licensed Social Workers in the State of Oregon.
  • Prefer one year of experience in hospice or home health.
  • Working knowledge of chronic illnesses and ability to explain major points to others.
  • Reliable transportation and auto insurance.
  • Successful completion of background check.
  • Ability to read, write, and understand the English language.
