Requirements:

Oregon LMSW (Masters-level Social Worker), CSWA (Clinical Social Work Associate), or Oregon LCSW (Licensed Clinical Social Worker) licensure required
Healthcare Provider Level American Heart Association BLS certification
Ability to pass Criminal Records Check and maintain a valid driver's license

Work Environment:
