Position Overview: In this clinical capacity, the Clinical Pharmacy Specialist II plays a vital role in the medication management system, delivering tailored pharmaceutical care to patients within designated areas.
Key Responsibilities:
- Provide comprehensive pharmaceutical care services, which encompass evaluating patient requirements, integrating age and disease-specific factors into medication therapy, and offering patient education.
- Adjust treatment plans based on patient responses and implement clinical interventions to identify, mitigate, and avert adverse medication events.
- Engage in clinical research and initiatives aimed at enhancing practice and ensuring compliance with patient care quality standards.
- Deliver educational training related to medication and pharmacy practices, while also serving as a mentor for Doctor of Pharmacy students and pharmacy residents.
Quality Management: The Clinical Pharmacy Specialist II will actively participate in quality management programs aimed at enhancing services through process monitoring, data analysis, and the implementation of effective interventions.
Clinical Management: Responsible for the clinical oversight of patients undergoing various specialty therapies for conditions such as Hepatitis C, inflammatory diseases, pulmonary disorders, neurological issues, dyslipidemia, osteoporosis, and oncology.
Pharmaceutical Care Services: Facilitate pharmaceutical care services and direct patient care initiatives to ensure that medication utilization aligns with patient needs, evidence-based practices, and regulatory requirements.
Practice Advancement: Identify practice-related challenges that necessitate evaluation and facilitate clinical research projects, quality improvement efforts, or healthcare provider education to advance pharmacy practice.
Patient Safety: Identify high-risk patients and implement strategies to enhance quality and safety in medication management.
Operational Support: Oversee the prior authorization and appeals processes, provide clinical support to pharmacy technicians, and conduct thorough patient assessments in accordance with established policies and accreditation standards.
Qualifications:
- Doctor of Pharmacy degree from an accredited institution is required.
- Completion of a PGY1 residency or a minimum of five years of clinical experience in community, ambulatory care, pharmacy benefit management, or hospital settings is necessary.
- Experience in specialty pharmacy is preferred.
- Familiarity with EPIC suite products is advantageous.
Expertise: A strong foundation in advanced pharmacy practice, specialty medication therapy management, and evidence-based medicine in relevant clinical areas is essential.
Licensure: Pharmacy preceptor license must be obtained within 90 days of employment.
This position is full-time and offers an opportunity to contribute significantly to patient care and pharmacy practice within CHRISTUS Health.
