Case Manager

Wheeler Clinic is seeking a highly skilled and compassionate Senior Counselor to join our team. As a Senior Counselor, you will provide direct services and case management to families, ensuring they receive the necessary support and resources to thrive.
Key Responsibilities:- Develop and implement case management plans to meet the unique needs of each family
- Provide direct services, including interventions and coordination of care, to promote healthy child development and family well-being
- Support group interventions and maintain accurate records of services provided
- Collaborate with internal and external providers to ensure seamless care coordination
- Identify and mitigate potential risk factors, promoting a safe and supportive environment for families
- Bachelor's degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, or a related field
- Previous experience working with early childhood and/or substance use is preferred
- Effective time management, strong verbal and written communication skills, and a valid driver's license are required
- Competitive hourly rate of $22.00
- Sign-on bonus of $2,000, with $1,000 paid at start and $1,000 paid at one year of service
