Nurse Practitioner or Physician Assistant

As a Senior Community Care Clinician with Optum Home and Community Care, you will work closely with primary care physicians to provide comprehensive care to patients in their homes and communities. Your role will involve collaborating with nursing staff and patients' families to ensure seamless care coordination and health outcomes.
Key responsibilities include:
Collaborating with case management teams to develop and implement care plans
Providing one-time comprehensive health assessments for patients in collaboration with the case management team
Documenting medical information in electronic medical records
Required Qualifications:
Certified Nurse Practitioner or Physician Assistant through a national board
Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
Active and unrestricted license to practice in the State of Connecticut
Preferred Qualifications:
1+ years of hands-on post-grad experience within Long Term Care
Understanding of Geriatrics and Chronic Illness
Proficient computer skills, including the ability to document medical information in electronic medical records
