Join our team as a Community Care Coordinator with Veterans Affairs, Veterans Health Administration As a Community Care Coordinator, you will play a critical role in ensuring that veterans receive timely and high-quality care in their communities. You will have the opportunity to make a real difference in the lives of our nation's heroes.

What We Offer
We offer a competitive salary, ranging from $70,000 to $90,000 per year, depending on experience and qualifications. In addition to the salary, you will receive a comprehensive benefits package, including paid time off, health insurance, retirement savings plan, and more.

Key Responsibilities
Your key responsibilities will include:
• Executing a streamlined approach to receiving, triaging, and directing timely, coordinated care
• Assessing, organizing, facilitating, and guiding veterans through their full range of care options, including internal/direct care in VA and care in the community
• Collaborating with members of the interdisciplinary team to coordinate care and manage complex client situations

Requirements
To be successful in this role, you will need:
• A graduate degree in nursing from an accredited institution
• Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia
• At least 2 years of specialized nursing experience equivalent to Nurse I, Level III; OR a Master's degree in nursing (MSN) and no additional professional nursing experience
