Licensed Practical Nurse

As a Licensed Practical Nurse (LPN) at Northern Rivers Family of Services, you will play a vital role in delivering comprehensive medical, dental, and mental health care to a diverse youth and adolescent demographic facing emotional, behavioral, and physical challenges. Your expertise will ensure that all medical services comply with regulatory standards while working collaboratively with a Registered Nurse on-site.
Key Responsibilities:
- Conduct nursing assessments and document findings for the supervising Registered Nurse.
- Analyze laboratory and diagnostic test results, promptly communicating critical information to the RN and healthcare providers.
- Manage medication processes, including ordering and securing prior authorizations.
- Administer prescribed medications to clients.
- Engage with medical directors and providers regarding client health observations and behavioral changes.
- Educate clients and their families about medications and treatment plans.
- Collaborate with interdisciplinary teams to address client concerns and behaviors.
- Participate in Interdisciplinary Medication Review meetings to ensure comprehensive care.
- Maintain accurate nursing documentation using Electronic Health Records (EHR).
- Assist in updating discharge planning requirements.
- Perform additional duties as assigned within the scope of practice and organizational needs.
- Current LPN licensure in New York State.
- 1 to 2 years of relevant nursing experience.
- Experience with youth and adolescent populations is preferred.
