The Clinical Nutrition Specialist evaluates the dietary needs of participants and utilizes clinical insights to devise and execute tailored nutrition strategies as an integral member of the Interdisciplinary Team. This role involves guiding the Dietary Aide, Food Service Manager, and Day Center CNA personnel in delivering exceptional food services to participants.
Key Responsibilities:
- Oversees food service operations and areas, providing guidance to staff to ensure compliance with quality regulatory standards.
- Collaborates with the food safety manager to guarantee meal safety.
- Partners with the Rehabilitation and Nutrition Services Manager to establish and enforce quality standards and protocols for nutritional services.
- Conducts educational sessions for PACE RI employees and participants on nutritional services.
- Maintains documentation in the EMR in line with CMS and PACE RI regulations.
- Performs comprehensive nutritional assessments for all PACE RI participants upon their enrollment.
- Actively participates in interdisciplinary team meetings, contributing to care planning and service allocation decisions.
- Works closely with the Dietary Aide and Day Center CNAs to oversee meal monitoring, including intake, diet adherence, supplements, and plate accuracy.
- Advises on and supervises the use of supplements in accordance with PACE RI guidelines.
- Analyzes data from food intake records and calorie counts, making recommendations for necessary adjustments to relevant staff.
- Designs and implements nutrition care plans for high-risk participants, including medical therapy recommendations.
- Monitors significant weight fluctuations and provides appropriate nutritional interventions.
- Assesses menus for nutritional adequacy across PACE centers.
- Reviews emergency food supplies for suitability and nutritional value at least annually.
- Educates participants and caregivers on dietary regimens and nutritional needs.
- May conduct home visits for family and participant education and menu planning.
- Contributes to the development and execution of the PACE RI Quality Assessment and Performance Improvement Plan for nutrition services.
- Keeps an updated diet list for dietary staff, making necessary changes and additions.
- Engages in additional duties/projects to support the department and participates in organizational committees.
- Maintains records for supplements, take-home meals, and dietary needs.
- Ensures the confidentiality and privacy of sensitive participant, employee, and agency information.
- Required to work at various PACE-RI sites and in community settings as necessary.
- Demonstrates the importance of thorough documentation as vital to quality participant care; ensures accurate and timely entry of participant documentation in the electronic health record.
- Performs other related duties as assigned.

Ability to exercise sound judgment under pressure and communicate effectively
Competent in performing clinical tasks within the scope of the position
Proficient in MS Office Suite or similar software
Educational Background:
Bachelor's Degree in Food and Nutrition or Dietetics, Required
Master's Degree, Preferred
Physical Demands:
Must be able to endure prolonged periods of sitting at a desk and working on a computer.
Must be capable of lifting up to 10 pounds, and occasionally up to 20 pounds.
Must navigate various departments within the organization and related community care settings.
Must tolerate conditions typical of a medical office and/or home care environment, including potential exposure to bloodborne pathogens and infectious diseases.
Experience:
2 Years of direct service work, Preferred
1 year of nutritional services experience with frail or elderly populations, Preferred
1 year of experience working with a frail or elderly population, Preferred
Licensure & Certification:
Rhode Island Dietitian/Nutritionist License (LDN)
Position Requirements:
American Dietetics Association Registration
Driver's License and access to reliable transportation.
