Position Summary:
Under the guidance of a Registered Nurse, the Continuous Care Licensed Practical Nurse (LPN) delivers nursing care and education to patients and their families who qualify for the Continuous Care Level of Hospice Services. The LPN may also undertake additional responsibilities to provide routine patient care in accordance with Arkansas Hospice, Inc. policies and State Department of Health regulations.
Qualifications:
- Education: Completion of an accredited practical nursing program.
- Licensure: Active Licensed Practical Nurse (LPN) license.
- Experience: At least one year of nursing experience; experience in hospice or home health preferred.
- Additional Requirements: Valid driver's license, personal vehicle, and automobile insurance.
Primary Responsibilities:
- Adhere to the Plan of Care for each assigned patient.
- Monitor patient conditions and assess care needs, applying nursing skills accordingly.
- Document patient conditions and care accurately, focusing on changes that require attention.
- Prepare clinical notes and summaries in collaboration with the RN supervisor.
- Assist patients with daily living activities while educating families and caregivers on safe home care practices.
- Ensure a safe environment for patients.
- Provide emotional and physical support to patients and families during the terminal phase of life.
- Work with physicians and registered nurses in complex care situations requiring continuous attention.
- Participate in interdisciplinary team meetings and training sessions as needed.
- Deliver care within the limits of training and the Scope of Practice as defined by the Arkansas State Board of Nursing.
- Perform assigned treatments as per the hospice's Interdisciplinary Plan of Care.
- Confirm patient schedules with the Inpatient Manager or Patient Care Coordinator to coordinate care visits.

- Perform other assigned duties as necessary.
