POSITION OVERVIEW

Under the guidance of a Registered Nurse, the Continuous Care Licensed Practical Nurse (LPN) delivers nursing care and education to patients and their families who qualify for the Continuous Care Level of Hospice Care. The LPN may also undertake additional responsibilities as needed to ensure routine patient care in accordance with organizational policies and state health regulations.

QUALIFICATIONS

Education: Completion of an accredited practical nursing program.

Licensure: Current Licensed Practical Nurse (LPN) license.

Experience: At least one year of nursing experience; experience in hospice or home health preferred.

Additional Requirements: Valid driver's license, personal vehicle, and automobile insurance.

PRIMARY DUTIES

  1. Adheres to the individualized Plan of Care for each patient.
  2. Monitors patient conditions and assesses care needs, applying nursing skills appropriately.
  3. Documents patient status and care accurately, noting any changes in condition.
  4. Prepares clinical notes and collaborates with RN supervisors for patient summaries.
  5. Assists patients with daily living activities while educating families on safe care practices.
  6. Ensures a safe environment for patients.
  7. Provides emotional and physical support to patients and families during end-of-life care.
  8. Works closely with physicians and RNs in complex care situations requiring continuous attention.
  9. Participates in interdisciplinary team meetings and training sessions.
  10. Delivers care within the scope of practice as defined by the nursing board.
  11. Administers treatments as per the individualized care plan.
  12. Confirms patient schedules with management to coordinate care visits.
PROFESSIONAL SKILLS AND COMPETENCIES

  1. Understanding of palliative care principles and symptom management for terminal illnesses.
  2. Knowledge of holistic patient care approaches and the nursing process in hospice settings.
  3. Familiarity with hospice philosophy and care principles.

  5. Capacity to recognize and address the emotional aspects of death and grief.
  6. Strong verbal and written communication skills.
  7. Ability to collaborate effectively with colleagues and other stakeholders.
