Role Summary:

Under the guidance of a Registered Nurse, the Continuous Care Licensed Practical Nurse (LPN) delivers nursing care and education to patients and their families who qualify for the Continuous Care Level of Hospice Services. The LPN may also undertake additional responsibilities to provide routine patient care, adhering to the policies and procedures of Arkansas Hospice, Inc. and the regulations set forth by the State Department of Health.

Qualifications:

  • Education: Completion of an accredited practical nursing program.
  • Licensure: Active Licensed Practical Nurse (LPN) license.
  • Experience: At least one year of nursing experience; experience in hospice or home health preferred.
  • Additional Requirements: Valid driver's license in Arkansas, personal vehicle, and automobile insurance.

Key Responsibilities:

  1. Adhere to the individualized Plan of Care for each patient.
  2. Monitor patient conditions and assess care needs, applying nursing skills effectively.
  3. Document patient status and care accurately, noting any changes in condition.
  4. Prepare clinical notes and summaries in collaboration with the RN supervisor.
  5. Assist patients with daily living activities while educating families and caregivers on safe home care practices.
  6. Ensure a safe environment for patients.
  7. Provide emotional and physical support to patients and families during end-of-life care.
  8. Work collaboratively with physicians and registered nurses in complex care situations.
  9. Participate in interdisciplinary team meetings and required training sessions.
  10. Deliver care within the defined scope of practice as established by the Arkansas State Board of Nursing.
  11. Perform assigned treatments as per the Interdisciplinary Plan of Care.
  12. Confirm patient schedules regularly with the Inpatient Manager or Patient Care Coordinator.
  13. Maintain confidentiality at all times.
  14. Exhibit excellent customer service skills and foster teamwork.
  15. Follow supervisor's directions and seek clarification when necessary.
  16. Comply with all organizational policies and procedures.
  17. Meet attendance and punctuality standards consistently.
  18. Attend all mandatory meetings and seek professional development opportunities.
  19. Engage actively in quality assurance and performance improvement initiatives.
  20. Perform additional duties as assigned.
