About the Role

We are offering a competitive salary of $65,000 per year, along with a generous sign-on bonus of $10,000. As an MDS Coordinator, you will be responsible for reviewing and processing Minimum Data Set (MDS) assessments for submission according to CMS and LTCI regulations.

Key Responsibilities
  • Initiate and participate in Interdisciplinary Care Plan meetings to ensure accurate and timely completion of the RAI/MDS.
  • Coordinate the development and revision of resident Interdisciplinary Care Plans based on information identified in the RAI/MDS.
  • Ensure that care plans address all triggered indicators and collaborate with nursing caregivers and other disciplines to obtain information on residents' status and progress toward care plan goals.
  • Demonstrate effective communication skills to work with various disciplines and complete the RAI/MDS accurately and efficiently.
  • Prepare reports on Quality Measure/Quality Indicators (QM/QI) derived from the RAI/MDS and track trends to enhance the knowledge of the Interdisciplinary Team and improve quality of care through performance improvement initiatives.
  • Be actively involved in Quality Improvement initiatives and implement educational plans to meet changing program or service needs.
