Nurse Practitioner Position Overview

Employment Type:

  • Full-Time Role: Monday to Friday, 8 am - 5 pm
  • Includes rotating weekends, holidays, and on-call support

Role Summary:

The Nurse Practitioner operates under the guidance of the Medical Director, delivering medical care and education to patients and their families across various environments, including inpatient, outpatient, home, and facility settings, focusing on both non-hospice palliative care and hospice services.

Key Responsibilities:

  • Educate patients and their families about care options.
  • Conduct patient visits for palliative care in diverse settings.
  • Provide hospice patient visits in both inpatient and outpatient environments.
  • Offer telephone support to palliative care patients during business hours.
  • Engage in Interdisciplinary Group (IDG) meetings.
  • Deliver community education on Hospice and Palliative Care as opportunities arise.
  • Contribute to the development of the Palliative Care program through exceptional service to patients, families, and referral sources.
  • Perform in-person assessments to confirm eligibility for hospice services.
  • Foster positive relationships with healthcare professionals in the community.
  • Ensure compliance with relevant regulations and standards for hospice and palliative care.
  • Participate in weekend and holiday on-call duties.
  • Achieve credentialing to provide care at partner facilities within four months of employment.
  • Obtain Certification in hospice and palliative care (CHPN) within one year of employment.

Qualifications:

  • Graduation from an accredited Nurse Practitioner program.
  • Valid Registered Nurse and Nurse Practitioner Licensure in the State of Michigan.
  • Preferred minimum of three years of experience as a Nurse Practitioner or equivalent clinical background.
  • Understanding of the principles and practices of hospice and palliative care.
  • Knowledgeable in the management of pain and symptoms in palliative care.
  • Strong communication skills and proficiency with mobile technology and electronic medical records.
  • Good standing as a Medicare and Medicaid Provider.
  • Possession of a valid State of Michigan driver's license.
  • Access to a reliable cell phone with local service.
