Position Summary:

The Nurse Practitioner, under the guidance of the Medical Director, delivers medical care and educational support to patients and their families across various environments, including inpatient, outpatient, home, and facility settings, specifically focusing on non-hospice palliative care and hospice services.

Key Responsibilities:

  • Educate patients and their families on care options.
  • Conduct patient visits for palliative care in both inpatient and outpatient environments.
  • Provide hospice patient visits across different settings.
  • Offer telephone support to palliative care patients during business hours.
  • Engage in Interdisciplinary Group (IDG) meetings.
  • Deliver community education on Hospice and Palliative Care as opportunities arise.
  • Support the development of the Palliative Care program through exceptional service to patients, families, and referral sources.
  • Perform assessments to confirm ongoing eligibility for hospice services.
  • Foster positive relationships with healthcare professionals in the community.
  • Ensure compliance with relevant regulations and standards for hospice and palliative care.
  • Participate in weekend and holiday on-call duties.
  • Achieve credentialing to provide care at partner facilities within four months of employment.
  • Obtain Certification in hospice and palliative care (CHPN) within one year of employment.

Qualifications:

  • Graduation from an accredited Nurse Practitioner program.
  • Current Registered Nurse and Nurse Practitioner Licensure in the State of Michigan.
  • Preferred minimum of three years of experience as a Nurse Practitioner or equivalent clinical background.
  • Understanding of the principles and practices of hospice and palliative care.
  • Knowledgeable in palliative medical management, particularly pain and symptom control.
  • Strong communication skills and proficiency with mobile technology and electronic medical records.
  • Medicare and Medicaid Provider status in good standing.
  • Valid driver's license in the State of Michigan.
  • Access to a reliable mobile phone with local coverage.
