LPN - Memory Care / Behavioral Specialist - Part-Time Evening Shifts

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at the Good Samaritan Society's Specialty Care Community in Robbinsdale, MN. As a LPN, you will play a vital role in providing high-quality care to our residents with memory care and behavioral health needs.
Key Responsibilities- Provide professional nursing care to residents of all ages in long-term care settings, under the supervision of a registered nurse, advanced practice provider, or physician.
- Assess residents' physical and psychosocial needs, develop and implement individualized care plans, and provide education to residents and their families.
- Administer medications and treatments as ordered, and document resident interactions and outcomes.
- Collaborate with interdisciplinary care teams to ensure comprehensive care and support for residents.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing board and/or possess multi-state licensure privileges.
- Obtains and maintains required department-specific competencies and certifications.
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility.
