The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician.
Key Responsibilities- Provides nursing care for residents in accordance with their individualized care plans.
- Administers medications and treatments as ordered by a physician or advanced practice provider.
- Conducts resident assessments and develops care plans to meet their physical and psychosocial needs.
- Collaborates with the interdisciplinary care team to ensure high-quality resident care.
- Documents resident interactions, outcomes, and care provided.
- Provides education to residents, family members, and caregivers on prevention and treatment plans.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing board and/or possess multi-state licensure privileges.
- Obtains and maintains required department-specific competencies and certifications.
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility.
