Job Title: Public Health Nurse

Houston County, MN is seeking a highly skilled and dedicated Public Health Nurse to join our team. As a Public Health Nurse, you will play a vital role in promoting health and well-being in our community.

Job Summary:

The Public Health Nurse will provide a range of services, including health education, disease prevention, and case management. You will work closely with community members, healthcare providers, and other stakeholders to promote healthy behaviors and improve health outcomes.

Responsibilities:
  • Provide health education and promotion to community members, including individuals, families, and groups.
  • Conduct health assessments and screenings to identify health risks and needs.
  • Develop and implement health plans to address identified health risks and needs.
  • Provide case management services to individuals and families, including connecting them with community resources and services.
  • Collaborate with healthcare providers and other stakeholders to promote healthy behaviors and improve health outcomes.
  • Participate in community outreach and engagement activities to promote health and wellness.
Requirements:
  • Master's degree in nursing or related field.
  • Current licensure as a registered nurse in Minnesota.
  • Experience in public health nursing or related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Preferred Qualifications:
  • Experience working with diverse populations, including low-income, minority, and rural communities.
  • Knowledge of community health assessment and planning principles.
  • Experience with health education and promotion programs.
