We are seeking a highly skilled Public Health Nurse to join our team at the Minnesota Merit System. As a Public Health Nurse, you will play a critical role in providing nursing care to individuals, families, and communities, and promoting health and wellness through education and outreach.
Key Responsibilities- Assess client needs and coordinate service providers to ensure comprehensive care.
- Provide direct skilled nursing services to clients as recommended by physicians.
- Conduct health assessments, identify health problems, and develop plans to address them.
- Teach clients and their families about healthy living, disease prevention, and management of chronic conditions.
- Collaborate with healthcare teams, community organizations, and other stakeholders to promote health and wellness.
- Develop and implement health education programs for communities and populations.
- Conduct home visits, school health services, and other community-based health initiatives.
- Certification as a Public Health Nurse in Minnesota and current licensure as a Registered Nurse.
- At least one year of experience as a Public Health Nurse, with a focus on community health and home health care.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a focus on collaboration and coordination.
