Care Coordinator for Children's Behavioral Health

We are seeking a highly skilled Care Coordinator to join our team at Spurwink Services. This role will involve working with children and families in our catchment area, based out of the Randolph office, which covers Kennebec County. The successful candidate will be part of a collaborative team that includes Care Coordinators, family support persons, clinical leads, and medical/psychiatric/and nursing supports.
Key Responsibilities:
- Manage a busy schedule and prioritize the needs of each family
- Coordinate with multiple providers to ensure the child's needs are being addressed
- Complete intakes, psychosocial assessments, and other intake assessments
- Develop and implement treatment plans, including initial and 90-day plans
- Provide regular contact with families and collaborate with providers
Requirements:
- BA in a related field
- Strong organization and communication skills
- Savvy computer skills to manage multiple systems
