Are you looking to practice rural medicine? Gifford has an opportunity for an experienced physician to join our well established, successful Primary Care office in Randolph. Gifford embraces a team approach to patient care, supported by collaborative relationships throughout the organization. As a primary care clinician, you will benefit from the additional support of our Community Health Team, and our Population Health Team for your patients.
Gifford Health Care, in Randolph, VT, an FQHC, along with Gifford Medical Center, a Critical Access Hospital is seeking a well-rounded Primary Care Physician.

As a Critical Access Hospital (CAH) and a Federally Qualified Health Center (FQHC), Gifford is well known for its progressive approach to providing primary, specialty, and surgical care to the many communities it serves. It is home to the first birthing center in Vermont, has modern imaging and diagnostic equipment, and an award winning nursing home and child enrichment center, as well as an outstanding food service run by top chefs. Duties and Responsibilities:
·Providing care to patients with recently discharged from a hospital stay or ED visit
·Educating patients and their families about their condition and what they should expect from the treatment plan
·Performing examinations and diagnostic test s to assess the patient’s condition
·Communicating with their Primary Care Clinician to determine the best care plan for every patient
·Providing acute care to patients as needed
·Keeping records that track patient progress
