Job Title: RN Field Team Supervisor

We are seeking a highly skilled and experienced RN Field Team Supervisor to join our team at Willcare Home & Community Based Services in Middletown, NY. This is a full-time position with an excellent schedule, offering a competitive salary of $80,000-85,000/year, a $5,000 sign-on bonus, and a comprehensive benefits package.

About the Role:

The RN Field Team Supervisor will be responsible for coordinating high-quality, cost-effective patient care, overseeing the operations and operational flow of the agency, and ensuring compliance with applicable laws and regulations. This role requires strong leadership and communication skills, as well as the ability to work effectively with healthcare professionals, patients, and families.

Key Responsibilities:

  • Provides high-quality clinical services within the scope of practice and infection control standards.
  • Completes clinical nursing assessments in accordance with federal and state program requirements.
  • Coordinates the plan of care with other healthcare professionals involved in patient care.
  • Ensures continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, case managers, and other community resources.

Requirements:

  • Current RN licensure in the state of practice.
  • Minimum one year of RN experience with a Certified Home Health Agency.
  • Current CPR certification.
  • Access to a dependable vehicle or public transportation.
