Job Title: RN MDS Case Manager

Providence Health & Services is seeking a skilled RN MDS Case Manager to join our team at Providence Mount St. Vincent. As a RN MDS Case Manager, you will play a vital role in ensuring the highest quality of care for our patients.

About the Role:

This is a part-time day shift position. The RN MDS Case Manager will be responsible for:

  • Conducting comprehensive assessments of patients to identify their needs and develop individualized care plans.
  • Collaborating with the interdisciplinary team to ensure seamless care coordination and communication.
  • Providing education and support to patients and their families to promote health and well-being.
  • Monitoring and evaluating patient outcomes to ensure quality and safety standards are met.
Requirements:

To be successful in this role, you will need:

  • A current Washington State Registered Nurse License.
  • At least two years of experience in Long-Term Care, Transitional Care Unit, or equivalent experience.
  • A Bachelor's Degree in Nursing is preferred.
