About the Role

We are seeking a skilled and compassionate RN MDS Case Manager to join our team at Providence Mount St. Vincent. As a member of our care management team, you will play a vital role in ensuring the highest quality of care for our patients.

Key Responsibilities
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes
Requirements
  • Current Washington State Registered Nurse License
  • At least two years of experience in long-term care, transitional care, or equivalent experience
  • Bachelor's Degree in Nursing (preferred)
