Position Overview

ROLE SUMMARY:

Under the general oversight of the Program Director and in alignment with the guidelines set forth by the New York State Office of Mental Health for the Mental Health Comprehensive Medicaid Case Management Program, this role is integral to a collaborative treatment team. The specialist will assist individuals facing serious and persistent mental health challenges in establishing and sustaining effective living, employment, and social environments within the community. This position holds primary responsibility for the integration of vocational objectives and services across the team’s activities. The specialist will also engage in crisis intervention services available to participants around the clock.

KEY RESPONSIBILITIES:
  • Lead the integration of vocational objectives and services within a multi-disciplinary team, delivering comprehensive support to adults with serious mental health conditions, including those with co-occurring disorders.
  • Conduct vocational assessments for assigned clients, facilitating job exploration aligned with their interests and strengths.
  • Engage in initiatives aimed at enhancing clients' abilities to choose, secure, and maintain employment.
  • Deliver counseling, rehabilitation, case management, and community support services to individuals with mental health challenges, particularly those with a history of treatment non-compliance.
  • Assist clients in achieving and maintaining stable housing, employment, and social connections while managing psychiatric symptoms and medication side effects.
  • Provide assertive engagement and mental health services in community settings, with a significant portion of the workweek dedicated to field activities.
  • Maintain continuous responsibility for individuals enrolled in the caseload.
  • Deliver all services with cultural relevance and competence.
  • Ensure accurate and timely maintenance of case records, including progress notes, service plans, assessments, and logs, in compliance with applicable regulations and guidelines.
  • Offer support and psychoeducation to family members and other significant individuals in the clients' lives.
  • Facilitate referrals and support for services not directly provided by the team, such as medical care and vocational training.
  • Prepare and submit timely reports to other service providers while adhering to confidentiality guidelines.
  • Participate in supervision, treatment team meetings, and professional development activities as required.
  • Comply with all relevant rules and regulations regarding incident reporting and client safety.
  • Perform additional duties as assigned by the Program Director.

QUALIFICATIONS:

A Master's Degree in a discipline recognized by the New York State Office of Mental Health as qualifying for professional clinical staff. A minimum of one year of experience in job placement, employment counseling, or vocational rehabilitation is required. A professional license in the relevant field issued by the New York State Education Department is highly preferred. Candidates must demonstrate proficiency in:

  • Developing skills for securing and maintaining employment.
