Job Title: Director of Case Management

Mass General Brigham is seeking a highly skilled and experienced Registered Nurse to lead our Case Management Program. As a Director of Case Management, you will be responsible for planning, directing, and managing the activities of the program to ensure high-quality, timely, and cost-efficient care for our patients.

Key Responsibilities:
  • Develop and supervise a team of RNs, SWs, and non-clinical support staff to ensure effective case management services.
  • Evaluate and maintain case management skills and outcomes to ensure compliance with key model components.
  • Manage all policies and procedures related to case management to ensure adherence to regulatory requirements.
  • Collaborate with the multidisciplinary team to assure quality, timely, and cost-efficient care.
Requirements:
  • Minimum 7 years acute clinical experience.
  • RN with Master's Degree.
  • Current License as Registered Nurse in the Commonwealth of Massachusetts.
  • Strong analytical abilities to strategically plan, consensus build, and problem solve.
  • Ability to communicate and convey complex information in a professional and courteous manner.
  • Administrative skills to supervise and develop staff, plan, organize, direct, and have proficiency in data analytics.
  • Strong understanding of clinical and financial outcomes.
  • Strong interpersonal skills to interact effectively with a wide range of personalities, ages, and diverse cultural backgrounds.
