Clinical Transition Educator

Soliant is seeking a skilled Clinical Transition Educator to join our team in Santa Monica, California.
This part-time opportunity requires a minimum of 1+ years of nursing experience and offers a competitive salary and benefits package.
As a Clinical Transition Educator, you will be responsible for evaluating, educating, and training patients, caregivers, and facility staff on the transition of patients to our care delivery model.
You will conduct patient assessments and evaluations to determine patient viability and partner with our sales team to understand clinical transition educational needs.
Maintaining confidentiality and observing legal guidelines for safeguarding patient and proprietary information is essential for this role.
