Job Overview:

New Transitions, Inc. is looking for a dedicated Licensed Practical Nurse (LPN) to become a vital part of our healthcare team. In this role, you will be responsible for developing and overseeing individualized care plans for patients. As an LPN, you will demonstrate exceptional attention to detail and a compassionate approach to patient care. Your ability to create tailored care strategies and effectively communicate with patients will be key to your success in this position.

Key Responsibilities:

Medical Record Management:

  • Establish and implement standardized protocols for maintaining medical records.
  • Ensure adherence to all regulatory standards and best practices.
  • Conduct regular audits of medical records to ensure accuracy and completeness.
  • Provide training to staff on documentation procedures and medical record systems.

Staff Development:

  • Educate staff on nursing best practices, patient care standards, and safety measures.
  • Design and facilitate training sessions for both new and existing personnel.
  • Offer continuous mentorship and support to nursing and direct care staff.
  • Assess the effectiveness of training initiatives and implement necessary improvements.

Patient Care and Oversight:

  • Perform thorough patient assessments and create personalized care plans.
  • Supervise the administration of medications and treatments.
  • Monitor patient progress and modify care plans as required.
  • Provide direct patient care when necessary.

Quality Improvement Initiatives:

  • Identify opportunities for enhancing patient care and operational efficiency.
  • Engage in quality improvement projects and implement changes as needed.
  • Ensure compliance with health and safety regulations.
  • Track and report on key performance indicators (KPIs) related to patient care and facility operations.

Communication and Team Collaboration:

  • Maintain effective communication with staff, patients, families, and healthcare professionals.
  • Collaborate with interdisciplinary teams to ensure comprehensive and coordinated patient care.
  • Participate in staff meetings, care conferences, and management discussions.

Qualifications:

  • Current licensure as a Licensed Practical Nurse (LPN) in MN
  • Prior experience as an LPN is preferred
  • Strong interpersonal and patient care skills
  • Valid driver's license
  • Certification in First Aid/CPR is required
