Position Overview:
The Long-Term Care (LTC) Registered Nurse plays a crucial role in providing comprehensive care to elderly patients, addressing both their physical and emotional well-being. This position requires a commitment to enhancing the quality of life for older adults through dedicated nursing practices.

Key Responsibilities:

  • Collaborate with healthcare professionals to assess, plan, implement, and evaluate individualized patient care strategies.
  • Administer and document prescribed medications, ensuring adherence to safety protocols and reporting any adverse reactions.
  • Monitor and record vital signs and medical histories of patients.
  • Evaluate patients' capabilities in performing daily activities and assist them with personal care tasks, promoting independence.
  • Identify and manage common geriatric syndromes, including cardiovascular, respiratory, and psychological issues.
  • Prepare necessary equipment and assist physicians during patient examinations and treatments.
  • Encourage active participation of older adults in their healthcare decisions.
  • Educate and involve family members in the care process, ensuring best practices are followed.
  • Advocate for quality end-of-life care options for elderly patients.
  • Maintain accurate and timely documentation of all patient care activities.
  • Perform additional duties as required based on the specific care setting.

Qualifications:

  • Graduate from an accredited nursing program.
  • Valid nursing license in good standing.
  • Minimum of one year of experience in Long-Term Care nursing within the last two years.
  • Current Basic Life Support (BLS) certification is mandatory.
