Nexus HR is seeking a highly skilled Registered Nurse to serve as a Clinical Manager for our reputable, locally owned and operated hospice company. The ideal candidate will provide exceptional care to terminally ill patients and their families, working closely with our interdisciplinary team to develop and implement comprehensive care plans.
Key Responsibilities- Supervise and coordinate the delivery of hospice care services, ensuring high-quality patient outcomes and satisfaction.
- Lead regular interdisciplinary team meetings to discuss patient care plans, address concerns, and make informed decisions.
- Assign patient cases to team members, ensuring optimal care and resource allocation.
- Monitor and maintain accurate medical records, adhering to licensing regulations, certification standards, and legal requirements.
- Provide clinical supervision and guidance to nursing personnel, Hospice Aides, and Homemakers.
- Conduct regular supervisory visits to assess team performance and provide constructive feedback.
- Develop and implement staff education programs to enhance skills and knowledge.
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care.
- Collaborate with the Administrator to develop and implement policies, procedures, and standards to ensure safe and effective patient care.
- Ensure compliance with federal, state, and local regulations, accreditation standards, and Hospice policies.
- Current licensure as a Registered Nurse in Georgia.
- Minimum 3 years of experience as a Registered Nurse, with a preference for hospice or home health experience.
- Valid driver's license and reliable transportation with appropriate insurance coverage.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to work in Atlanta, GA or Alpharetta, GA.
