Role Summary:
The Patient Care Coordinator (PCC) plays a crucial role in managing and coordinating care for assigned patients, aiming to achieve favorable clinical outcomes and financial efficiency. The PCC collaborates closely with healthcare providers, nursing staff, and other team members to implement interventions that support positive patient outcomes. This role includes developing and executing discharge plans, collaborating with social workers, and addressing legal and psychological factors that may affect patient care. The PCC ensures that discharge plans are prepared in alignment with the patient's medical readiness for transition to subsequent care levels.
Key Responsibilities:
1. Daily Patient Care Coordination: Manage and facilitate care for assigned patients, ensuring:
a. Prioritization of patients and assessment of new admissions.
b. Implementation of physician orders.
c. Documentation of nursing and ancillary interventions.
d. Timely communication of key outcomes and progress to physicians.
e. Prompting physicians for necessary orders related to patient care and discharge.
f. Streamlining physician communication to minimize interruptions while ensuring timely interventions.
g. Establishing anticipated discharge dates based on physician input to ensure readiness for discharge.
h. Preparing discharge plans in advance of medical readiness.
i. Securing authorizations or referrals for ongoing care.
2. Interdisciplinary Collaboration: Lead daily rounds with interdisciplinary teams to review patient progress and identify discharge barriers.
3. Patient and Family Engagement: Assist in conducting patient/family interviews and care conferences.
4. Monitoring and Validation: Oversee patient status for assigned populations, including collaboration with Access Nurses and Utilization Management staff.
5. Documentation: Ensure timely and comprehensive documentation of care activities in patient medical records.
6. Quality Improvement: Collect data related to core measures, readmissions, and discharge delays to support continuous improvement initiatives.
7. Facilitation of Care Paths: Collaborate in the development of care pathways and educational materials to enhance patient care efficiency.
8. Professional Development: Stay informed on case management and discharge planning practices through ongoing education and participation in professional organizations.
9. Flexibility: Adapt to departmental needs, providing coverage as required throughout the week.
Qualifications:
Education: Graduate of an accredited nursing program with a Bachelor's Degree in Nursing.
Licensure: Current Registered Nurse licensure in North Carolina.
Experience: Minimum of 3 years of clinical experience as a Registered Nurse, with at least 1 year in hospital case management preferred.
Skills: Strong knowledge of the healthcare reimbursement landscape, excellent interpersonal skills, and proficiency in MS Office and EPIC software.
