Job Title: Community Case Manager

Abraxas Youth & Family Services is seeking a qualified Community Case Manager to join our PATH Program in Reading, PA.

Job Summary:

The Community Case Manager will assist court-committed youth and their families with the transition from residential placement back to the community. The ideal candidate will have a strong understanding of the juvenile justice system and be able to develop and maintain relationships with community service sites and assigned Juvenile Probation Department.

Responsibilities:
  • Maintain a client caseload with appropriate service plans
  • Develop and maintain relationships with community service sites and assigned Juvenile Probation Department
  • Orient clients to program norms, rules, and behavioral standards
  • Collect YLS assessments from the Juvenile Probation Department and implement them into service plans and delivery
  • Build and maintain positive relationships with youth and their families through caring, consistent, and competent support
  • Support clients and their families in completing court-ordered conditions, including restitution, community service, or specific counseling needs and assessments
  • Supervise client activities and enter notes into the Echo Advantage System bi-weekly
  • Communicate with clients' Probation Officers on a bi-weekly basis
  • Maintain receipt books for restitution payments and make timely payments to the Program Specialist or Clerk's Office
Requirements:
  • High school diploma and minimum of three years of related experience in the juvenile field
  • Bachelor's degree in a related field preferred and minimum of one year of related experience in the juvenile field
  • Master's degree in a related field and minimum of six months of related experience in the juvenile field
  • At least 21 years of age
  • Driver's License
  • Physical exam with TB and drug testing
  • Criminal clearances (State Police, FBI, and State Child Abuse Clearances)
  • Satisfactory completion of background screening and applicable pre-employment checks
