Abraxas Youth & Family Services is seeking a skilled Community Case Manager to join our PATH Program in Reading, Pennsylvania.
Job Summary:The Community Case Manager will work with court-committed youth and their families to provide in-home services and support to help them transition back to the community or avoid out-of-home placement.
Key Responsibilities:- Maintain a client caseload and develop individualized service plans.
- Build and maintain relationships with community service sites and assigned Juvenile Probation Department.
- Orient clients to program norms and rules, and collect and implement YLS assessments.
- Support clients in completing court-ordered conditions, including restitution and community service.
- Supervise client activities and maintain accurate records in the Echo Advantage System.
- Communicate with Probation Officers and make timely payments of restitution.
- High school diploma and minimum of three years of related experience in the juvenile field.
- At least twenty-one (21) years of age.
- Driver's License.
- Physical exam, TB, and drug testing.
- Criminal clearances (State Police, FBI, and State Child Abuse Clearances).
- Ability to work with computers and necessary software.
