Job Title: Case Manager

At Safe Berks, we are committed to providing a safe haven and support for victims of domestic violence and sexual assault. We are seeking a highly skilled and compassionate Case Manager to join our team.

About the Role

The Case Manager will provide trauma-informed case management services to survivors and their children in our Emergency Shelter Program. This includes developing a goal plan that regularly assesses their safety and well-being, providing referrals and assistance with connecting to all available Safe Berks services and community resources, and coordinating exit plans for survivors leaving the program.

Responsibilities
  • Meet with survivors and their children upon entry into the Emergency Shelter Program
  • Provide ongoing quality, trauma-informed case management services to survivors and their children
  • Assist survivors with exploring/establishing/maintaining a continuum of services, including employment, other income eligibilities, budgeting skills, school enrollment, securing childcare, connections to medical, mental health, and substance use disorder resources as applicable, education for basic living/parenting skills, locating/securing permanent housing, etc.
  • Provide referrals and assistance with connecting to all available Safe Berks services and community resources
  • Initiate referrals for school-age children to the IU under the McKinney-Vento Act, when appropriate
  • Assist survivors with safety planning, identifying supports, and goal planning
  • Coordinate exit plans for survivors leaving the Emergency Shelter Program
  • Explore community resources and develop/strengthen community partnerships with other social service agencies
  • Maintain accurate and comprehensive case notes and electronic records in accordance with agency standards
  • Attend and actively participate in individual/group supervision and staff meetings
  • Crosstrain with Crisis Hotline & Intake Advocates
  • Crosstrain with Residential Services Advocates
  • All other duties as determined by department director
Requirements
  • 2+ years of experience in the social or human services field
  • Prior experience providing case management to underserved communities required
  • High school diploma or equivalent degree required
Working Conditions

This position requires the ability to work in a fast-paced environment and to prioritize multiple tasks and responsibilities. The Case Manager must be able to maintain confidentiality and handle sensitive information with discretion.
