Key Responsibilities

Enhabit Home Health & Hospice is looking for a qualified Registered Nurse (RN) or Physical Therapist (PT) to fulfill the role of Care Transition Coordinator.

Key responsibilities include:

  • Guiding patients through the complexities of post-acute care.
  • Assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to ensure a safe transition from acute care to home health or hospice services.
  • Utilizing evidence-based clinical guidelines and protocols to develop patient-centered transition plans that enhance quality and efficiency in post-acute care delivery.
  • Encouraging adherence to post-acute care plans and ensuring the completion of ordered services.
  • Representing Enhabit in transitional care initiatives and fostering strategic partnerships with health systems, hospitals, and physician groups.
  • Overseeing the execution of transitional care services through regular quality assurance visits with referral sources.
  • Meeting or exceeding referral and admission targets.
  • Acting as a clinical liaison responsible for care transitions program admissions, positively influencing patient outcomes and referral source satisfaction.

Qualifications

The ideal candidate will be a goal-oriented Registered Nurse (RN) or Physical Therapist (PT) with a strong sales motivation and prior experience in home health or hospice settings.

  • Must be a graduate of an accredited nursing or therapy program and hold a valid license in the state of employment.
  • Minimum of 2-3 years of field experience is required.
  • Strong understanding of customer and market dynamics, along with transitional care best practices.
  • Familiarity with Federal, State, and local regulations governing home health and hospice operations.
  • Excellent communication skills with the ability to engage effectively with diverse individuals.
  • Experience in territory management, strong presentation skills, performance management, relationship building, and sales planning.
  • Self-starter who can work independently with minimal supervision.
  • Previous experience in home health or healthcare sales is preferred.
