About the Role

This Care Manager and Discharge Planner position plays a vital role in ensuring the continuity of patient care throughout all healthcare settings. The successful candidate will work closely with clinical teams to develop and implement individualized treatment plans, utilizing their expertise to navigate complex patient needs.

To excel in this role, you will require 1 year of directly related experience, a strong understanding of patient-centered care principles, and excellent communication skills.

Essential Skills and Qualifications:
  • RN licensure in NM or as allowed by reciprocal agreement by NM
  • CPR for Healthcare/BLS Prov or Prof Rescuers certification
  • Strong understanding of patient-centered care principles
