We are seeking an experienced Registered Nurse to join our team as a Senior Patient Care Coordinator. This is a per diem position that offers flexible scheduling and the opportunity to work with a diverse patient population.

The estimated annual salary for this position is $85,000 - $110,000 based on experience.

About the Job

As a Senior Patient Care Coordinator, you will be responsible for coordinating all systems and services required for an organized, multidisciplinary, patient-centered care team approach. You will assure quality, cost-effective care for the identified patient population by managing the course of treatment, coordinating care with physicians, nurses, and other staff, and ensuring quality patient outcomes within established time frames and with efficient utilization of resources.

Key Responsibilities:
  • Conduct initial and ongoing assessments, initiate disease management protocols, determine and manage outcomes, ensure continuity of care through discharge planning, utilization of resources, and analysis of variances.
  • Function as a contact person for patients, families, healthcare team members, community resources, and employees as necessary.
  • Ensure adherence to Hospitals and departmental policies and procedures.
  • Patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups.
Qualifications:
  • Bachelor's Degree in Nursing or equivalent experience.
  • Two years directly related experience as a Registered Nurse.
  • RN licensure in New Mexico or reciprocal agreement.
  • CPR for Healthcare/BLS Provider or Professional Rescuer certification.
Working Conditions:

This position involves Light Work, exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly.
