About the Role

As a Multidisciplinary Care Team Member, you will work closely with our interdisciplinary team to achieve quality outcomes for patients within our local communities. You will be responsible for coordinating all systems and services required for an organized, multidisciplinary, patient-centered care team approach.

Key Responsibilities
  • Coordinate patient care throughout all healthcare settings, promoting effective communication among healthcare team members, including patients, families, and significant others.
  • Assess patient clinical, psychosocial status, and current treatment plans, identifying areas for improvement and developing collaborative relationships with other departments and community health care agencies.
  • Participate in continuous quality improvement activities, evaluating patient care systems that may include standards, protocols, and documentation.
Salary Range

We offer a competitive salary range of $90,000 - $120,000 per year, based on experience.
