About the Role

We are seeking a highly skilled and compassionate Patient Care Navigator to join our team at Field of Talent. As a Patient Care Navigator, you will play a vital role in guiding patients through their healthcare journey, ensuring a seamless and personalized experience.

Key Responsibilities
  • Serve as the primary point of contact for patients, providing guidance and support throughout their care journey
  • Work closely with clients to help them make informed decisions about their healthcare
  • Educate patients on our services and promote wellness through holistic and natural approaches
  • Conduct pre-appointment calls to gather patient information and set expectations
  • Follow up with patients post-appointment to ensure they understand their treatment plans
  • Provide information and recommendations to help patients make informed decisions about their care
Administrative and Team Coordination
  • Manage patient records and ensure all necessary documentation is obtained
  • Coordinate with our offshore team members to handle administrative tasks
  • Assist with various office tasks as needed, demonstrating a team-oriented attitude
  • Collaborate with the medical assistant and other team members

Requirements
