Job Summary

As a Transitional Care Unit Registered Nurse, you will provide direct and indirect patient care in a dynamic and supportive environment.

Key Responsibilities
  • Communicate effectively with physicians, managers, and colleagues to ensure seamless patient care.
  • Assess and respond to changes in patient condition, including results of diagnostic studies and symptomatology.
  • Perform general nursing duties with adequate supervision, prioritizing patient safety and well-being.
  • Evaluate patients for fall risk and implement appropriate protocols.
  • Develop and implement individualized plans of care, incorporating evidence-based practices and patient-centered approaches.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Document patient information accurately and efficiently, adhering to hospital policies and standards.
Requirements
  • Current unrestricted Wyoming Registered Nurse licensure.
  • Rehab nursing experience preferred.
  • BLS certification required.
  • Central Registry clearance required.
