Sheridan Memorial Hospital is a state-of-the-art facility nestled beneath the Big Horn Mountains in beautiful Northeast Wyoming. Founded over 100 years ago, Sheridan Memorial is licensed for 88 beds with over 60 physicians who provide primary medical care and specialty care in more than 15 areas.
Job SummaryProvides direct and indirect patient care in the transitional care setting. Communicates with physician/Manager/co-workers, as appropriate about changes in patient's clinical condition including results of diagnostic studies and symptomatology.
Essential Job Duties/Responsibilities- Knowledge of medications and their correct administration based on age of the patient and their clinical condition.
- Performs patient care responsibilities specific to the standard of care for patient's age.
- Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.
- Able to interpret data about the patient's status in order to identify each patient's age specific needs and provide care needed by the patient group.
- Ability to perform a head-to-toe assessment on all patients and reassessments as per policy and specific to unit age specific populations.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques including non-pharmacological techniques. Educates the patient and family regarding pain management.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Ability to formulate and revise an individualized plan of care as indicated by the patient's response to treatment. Evaluates overall plan daily for effectiveness.
- Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Family is included in teaching as appropriate.
- Demonstrates ability to perform treatments and provide services within scope of practice.
- Communicates appropriately and clearly to all health care team members.
- Consults other departments as appropriate to provide for an interdisciplinary approach to the patient's needs.
- Provides care appropriate to disease condition and age of the patient.
- Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills.
- Demonstrates an ability to be flexible, organized and function well under stressful situations.
- Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
- Functions as liaison between the patient/family, physicians, other healthcare providers and administration.
- Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
- Maintains a good working relationship both within the department and with other departments.
- Documentation meets current standards and policies.
- Identifies and addresses psychosocial needs of patients and family; communicates with social service/discharge planner regarding both in hospital and post discharge needs.
- Coordinates and supervises patient care as necessary.
- Respects patient privacy in accordance with Health Insurance Portability and Accountability Act.
- Participates in interdisciplinary care team meetings, department huddles and care conferences as scheduled.
- Transitional Care Specific
- Evaluates patients identifying those at risk for falls and implements the appropriate protocols.
- Exhibits appropriate use of the nursing process: assessment, planning, intervention and evaluation of the following disease entities: COPD, pneumonia, diabetes, anticoagulation therapy, total knee/hip replacement, cholecystectomy, CHF, UTI, MRSA infections, Detox, cancer, pain management.
- Participates in departmental performance improvement activities.
- Demonstrates knowledge of standards for restraints and seclusion, i.e. Restraint and Seclusion protocol, physician orders, documentation and reporting, and patient rights and teaching.
- Knowledgeable of Infection Control, TB, Blood borne Pathogens and Hazardous waste plans.
