About the Role
Enhabit Home Health & Hospice is seeking a Licensed Master Social Worker (LMSW) to provide comprehensive, family-oriented services to individuals in the home setting.
Key Responsibilities
- Assess patients' and families' psychosocial, environmental, and financial needs.
- Develop and implement a plan of care in collaboration with patients, families, and other caregivers.
- Provide case management and support as needed.
- Maintain accurate documentation in patient records.
- Monitor and evaluate changes in patients' conditions and environments.
Requirements
- Must be a Licensed Master Social Worker (LMSW) in the state of employment.
- Must have a Master's degree from a school of social work accredited by the Council on Social Work Education.
- Must possess one year of social work experience in a healthcare setting.
- Medicare home health or hospice experience is preferred.
