Medical Social Worker

We are seeking a highly skilled Medical Social Worker to join our team at Tuality Healthcare. As a Medical Social Worker, you will play a vital role in providing psychosocial support to patients and their families, ensuring they receive the best possible care.
Key Responsibilities- Assess patients' psychosocial needs and develop individualized care plans to address their unique challenges.
- Collaborate with healthcare teams to provide comprehensive care and support to patients and their families.
- Provide education and resources to patients and families on advanced directives, power of attorney, and guardianship.
- Respond to outpatient and community questions and referrals, ensuring timely and effective support.
- Collect and analyze data to identify trends and areas for improvement in patient care.
- Develop and implement behavioral management plans for patients with challenging behaviors.
- Participate in daily behavioral health rounds with the psychiatric and behavioral health team.
- Refer patients and families to community agencies and resources, fostering strong partnerships and connections.
- Provide social work services for terminal care, including supportive counseling and referrals to community resources.
- Facilitate discharge planning for patients with acute or persistent psychiatric needs.
- Assess for possible child, elderly, or domestic abuse, reporting and referring as necessary.
- Perform psychosocial assessments and make treatment recommendations and referrals.
- Provide resource referrals for individuals with financial concerns.
- Assist with complicated discharge planning and coordinate referrals for county, state, and federal agencies.
- Master's degree from an accredited school of social work or related field.
- Experience working in highly confidential settings.
- Knowledge of chronic, acute, and terminal illness and physical disabilities, as well as acute and chronic mental health/illness in adults, adolescents, and children.
- Adherence to the National Association of Social Workers Code of Ethics.
- Knowledge of community health and social service resources.
- Excellent customer service skills and ability to work with diverse populations.
- Ability to write clear and legible clinical notes and communicate effectively with patients, families, and healthcare teams.
- Familiarity with computerized word processing, database, and spreadsheet programs, as well as EMR systems.
- Self-directed and able to follow through on projects with attention to detail.
