About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Almost Family, part of LHC Group. As a Home Care LPN, you will have the opportunity to provide high-quality clinical services to patients in their homes, while enjoying a flexible schedule and opportunities for career growth.

Key Responsibilities
  • Provide direct nursing care to patients in their homes, as prescribed by physicians.
  • Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
  • Work collaboratively with the healthcare team to develop and implement patient care plans.
  • Ensure confidentiality of patient health information and maintain accurate records.
Requirements
  • Current LPN licensure in the state of practice.
  • Minimum of one year of acute care experience, with previous home care experience preferred.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR certification required.
