Key Responsibilities- Provide Direct Patient Care: Deliver high-quality clinical services to patients in their homes, following the plan of care and in coordination with the healthcare team.
- Document Patient Visits: Accurately document patient visits and progress in the electronic medical record, adhering to regulatory and payer requirements.
- Participate in Care Coordination: Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other healthcare professionals.
- Ensure Patient Confidentiality: Maintain the confidentiality of patient health information, adhering to agency policies and procedures.
- Education: Meet the educational and experience requirements outlined in the laws of licensure states.
- Experience: A minimum of one year of acute care experience is preferred, with previous home care experience also preferred.
- License: Must have current LPN licensure in the state of practice, as well as a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Certifications: Current CPR certification is required.
- Strong Patient Management Skills: Ability to relate to and treat acutely ill/chronically ill patients/families and their particular problems.
- Excellent Communication Skills: Possess strong communication and documentation skills, with the ability to interact professionally as a member of a multi-disciplinary team.
- Flexibility and Adaptability: Ability to work flexible hours as required to meet identified patient needs, with a strong commitment to the goals, mission, and philosophy of the organization.
