About the Role

We are seeking a skilled Registered Nurse to join our team in providing high-quality care to patients in their homes. As a Registered Nurse in Home and Community Based Services, you will have the opportunity to build trusted relationships with patients and their families, while also enjoying a flexible work schedule and opportunities for career growth.

Key Responsibilities
  • Provide high-quality clinical services to patients in their homes, in accordance with the plan of care and within infection control standards.
  • Conduct clinical nursing assessments to ensure patient eligibility and medical necessity for services.
  • Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements.
  • Ensure plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.
Requirements
  • Current RN licensure in the state of practice.
  • Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
Preferred Qualifications
  • A minimum of one year experience as an RN.
  • Recent clinical experience as an LPN may be considered in lieu of RN experience.
