Social Worker MSW

We are seeking a highly skilled and compassionate Social Worker MSW to join our team at Medical City Dallas. As a key member of our healthcare team, you will be responsible for providing non-clinical, biopsychosocial assessments and interventions to patients and their families. Your expertise will help identify and address barriers to discharge and recovery, ensuring a smooth transition to post-acute care.
Key Responsibilities- Collaborate with RN case managers, physicians, and other staff members to proactively identify and address patient needs.
- Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
- Provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.
- Coordinate and facilitate meetings between the care team and patients/families/caregivers.
- Engage in community outreach and program development to support patients/families/caregivers.
- Master's Degree in Social Work required.
- Currently licensed as Registered Master of Social Work in the state of TX.
- At least 1 year Acute care social work experience required.
- Hospital discharge planning and medical social work experience working with women and children population.
- Knowledgeable about internal, community, and post-acute care resources.
