About the Position:
This is a PRN/Part Time opportunity to work as a Nurse Practitioner or Physician Assistant in a variety of settings, including home visits, skilled facility visits, and telehealth visits.
As a member of our team, you will have the opportunity to work with a diverse range of patients and develop your clinical skills in a fast-paced and dynamic environment.
You will be responsible for conducting comprehensive health assessments, identifying potential health risks, and developing personalized care plans to help patients achieve their health goals.

Responsibilities:
- Conduct comprehensive health assessments, including medical history, diagnosis, and treatment.
- Identify potential health risks and develop personalized care plans to help patients achieve their health goals.
- Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to deliver high-quality care.
- Provide education and support to patients and their families to help them manage their health and achieve their goals.
- Document patient information and care plans accurately and thoroughly.
Qualifications:
- Masters Degree in Nursing or related field required.
- Current RN and NP or PA licensure in state of practice required.
- Board certified by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty required.
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant required.
- Strong computer skills and familiarity with EMR software required.
- Excellent verbal and written communication skills required.
Travel Requirements:
This position requires the ability to travel to patients' homes and communities, and may require occasional overnight stays.
A valid state driver's license and adequate automobile insurance coverage are required.
